HCH Joins in Celebrating the Opening of SCAN’s Clinic at Clearwater at South Bay

On May 2, 2024, Healing Care Hospice joined our colleagues on the SCAN Embrace team to celebrate the grand opening of SCAN’s new clinic at Clearwater at South Bay. The clinic will be spearheaded by Nurse Practitioner, Henry Hueng.
SCAN is one of the nation’s largest non-profit Medicare Advantage health plans, providing senior care in Southern California and other regions across the country. Their goal is to improve seniors’ access to care and connect them with the elderly care services they need to sustain a healthy and independent lifestyle.
Healing Care Hospice – Hospice Care Los Angeles has been partnering with SCAN Embrace for the past year and a half under a specialty program initiated by the Value-Based Insurance Design (VBID) Model. In accordance with the VBID Model, SCAN vetted a multitude of hospices to work in conjunction with them to manage their patient population in Southern California.
Healing Care Hospice was one of seven hospice care services providers chosen to partner with SCAN’s specialty iSNP Program, SCAN Embrace.
SCAN Embrace is a special-needs program that focuses on seniors that live at assisted living, memory care, board and care, residential care, and skilled nursing facilities. Their skilled medical team visits patients and provides wholesome elderly care to keep seniors healthy and prevent them from suffering serious illnesses that cause them to frequent acute settings.
NP Hueng and his SCAN Embrace team at Clearwater have built a reputation on prioritizing patient care. Thus, it comes as no surprise that they are the preferred medical group at Clearwater at South Bay. This partnership created the opportunity for SCAN to open a clinic at Clearwater. The clinic will be open to all SCAN patients, but those at Clearwater at South Bay will now have the advantage of having quality patient care under their own roof.
The SCAN Embrace team works closely with HCH Account Managers, Joe Rodriguez, Adriana Brunone, Joy King, Nancy Colon-Loera, and Elizabeth Gomez, connecting our team with SCAN patients and families on their end-of-life progress and decline. We look forward to what the future holds for HCH’s blossoming partnership with our colleagues at SCAN Embrace.
